Resurrection Home Health Services is hiring a
Physical Therapist on a Registry (as needed) basis.
Duties and responsibilities: -
Plan and implement specific treatment/programs for home health patients according to principles and practices of physical therapy.

Evaluate patients' functional ability using appropriate tests.

Develop specific physical therapy goals to be included in the Plan of Care.

Administer and instruct prescribed physical therapy treatments to restore functions, relieve pain, and prevent further disease or injury.
Qualifications: -
Bachelor's degree in Physical Therapy.

Current Illinois license in physical therapy.
-
CPR certification.
-
Valid Illinois driver's license & proof of current auto insurance.

1-2 years experience.

Good communication skills.
-
Extended periods of standing and walking; ability to lift and move 5-100 lbs and bending during work shift.

Must be able to provide own transportation to patients' homes.
