Complete Description

RESPONSIBLE FOR: Participates in the development, implementation, and maintenance of the left ventricular assist device (LVAD) program and the heart transplant program; to coordinate and manage the patient care of this high-risk patient population throughout the continuum; to provide the communication link for coordination of care among the multidisciplinary health care team involved with this patient type, to maintain a performance improvement program that focuses on continually improving the outcomes of these patients; to receive, evaluate and summarize organ donor information for transplant recipients from UNOS and LifeLink of Georgia; to provide data management of patients from evaluation, surgical intervention and throughout the remainder of the patients life; to provide education and mentorship to the care team and Nursing Department; to assists in the allocation of available deceased donor organs for heart transplant patients. Requires night and weekend donor call for heart transplant service. Patient care includes care to patients in the following age groups: adult (18 through 64 years) and geriatric (65 years and above).

QUALIFICATIONS: Graduate from an accredited school of nursing with a current R.N. license in the state of Georgia. ACLS certification required. Five (5) years of professional clinical experience with cardiovascular focus preferred. One (1) year experience caring for LVAD patients and heart transplant patients preferred. Leadership qualities including time management, verbal and good written communication skills, listening skills, decision making, priority setting and work organization skills. Must be self-directed and use independent judgment. Effective communication skills and team building skills required. Demonstrate compassionate patient communication.
